Whatever you are doing, put it into this context. In practical terms this means capturing and updating risk, assumptions, dependencies, issue and opportunity logs at least once a day. As you make notes in meetings think in these terms, and update the logs shortly afterwards. Immediately analyse these and take action! Do not leave this to the next day.
